Health Access Project works with our Volunteer Provider Network to connect you to the care you need.

CASE MANAGEMENT SERVICES

The population of Salt Lake County is becoming increasingly diverse. In addition to financial barriers, ethnic and racial minorities face linguistic and cultural barriers that make it difficult to access health care services. Health Access Project (HAP) addresses these barriers by maintaining bilingual, culturally-sensitive staff and by providing interpretation services for clients with limited English proficiency. HAP recruits volunteers to assist in these efforts.

Every HAP client is assigned a case manager who works closely with them and their providers to coordinate and arrange health care services they are in need of, as well as other services, such as interpretation services, transportation, and assistance in applying for public health insurance. HAP case managers screen clients for eligibility in receiving donated care, as well as other benefits they may receive. They schedule doctor appointments to address clients’ needs and coordinate follow-up care in facilitating communication between clients and providers.

COORDINATED HEALTHCARE

Clients of Health Access Project (HAP) receive charitable health care services donated by members of our HAP Volunteer Provider Network, a group of over 600 volunteer providers, each of whom have pledged to provide charity care in their own offices to our clients. Participating volunteer providers include: Physicians, Nurse Practitioners, Physician Assistants, Nurse Midwives, Radiologists, Anesthesiologists, Dentists, and Hygienists. These providers work out of hospitals, private clinics, and public clinics.

To allow clients access to a full range of healthcare services, HAP has facilitated charity agreements with all hospitals in the Salt Lake Valley, 2 national labs, and various outpatient care centers. These agreements are specific to HAP clients and allow for prompt access to services by our volunteer provider network. In order to qualify for charitable services through HAP, clients must have an annual income less than or equal to 150% of the Federal Poverty Guidelines, reside in Salt Lake County, and be uninsured or underinsured. 

HEALTH INSURANCE ASSISTANCE

An integral function of Health Access Project (HAP) is to assist our clients in navigating our healthcare system and aid them in applying for, receiving, and using public health insurance, such as Medicaid, Medicare, and CHIP. The Affordable Care Act (ACA or Obamacare) has allowed HAP to expand our scope of service to assist individuals and families in applying for health insurance through the Marketplace. To aid our clients and consumers in applying to public health insurance, HAP employs Certified Application Counselors (CAC), who are trained and able to help those in need look for health coverage options.

Our CAC assist clients and consumers with navigating our healthcare system, answering questions regarding eligibility for public health insurance, and enrolling into qualified health insurance programs. HAP CACs are experts in guiding eligible clients and consumers through the application process from beginning to end, and identify resources that enhance healthcare delivery services. Their services are free to clients and consumers alike. 

INTERPRETATION SERVICES

Clients are referred to HAP from a variety of sources including hospital emergency rooms, Salt Lake Valley safety net clinics, our volunteer providers, and other partnering agencies. In order to assist our culturally diverse clients, Health Access Project (HAP) maintains bilingual, culturally-sensitive staff in addressing client linguistic and cultural barriers. We also provide interpretation services for clients with limited English proficiency. HAP recruits volunteer interpreters to assist in these efforts.

HAP volunteer interpreters speak a number of different languages, including Spanish, French, Swahili, Russian, and Mandarin, just to name a few. Interpretation services are provided free of charge for our clients at their healthcare appointments. Clients of HAP may also be eligible for other services, such as prescription assistance and or transportation to and from their arranged healthcare appointments through HAP.
